Core OT is a mobile occupational therapy service for clients leaving hospital or living at home with aged care or NDIS support across Brisbane, Moreton Bay and Logan. We visit the client at home, assess what they need to be safe and independent, and send you a clear written report with recommendations. New referrals are usually seen within about one week.

For hospital discharge planners and social workers

When a patient is going home, we can see them in their own home soon after discharge and check that the home and supports match their needs.

• Home safety and access assessments after discharge
• Falls risk review and falls prevention strategies
• Equipment assessment, trial and prescription (shower chairs, toilet frames, mobility aids, beds and transfer equipment)
• Home modification recommendations (grab rails, ramps, bathroom changes, step and access changes)

For aged care providers, care managers and OTs

If your organisation delivers Support at Home (formerly Home Care Packages) and needs an occupational therapist for your clients, Core OT can be your mobile OT. We work alongside your care managers, keep you updated, and provide reports you can use for planning and funding decisions.

Brisbane North and inner Brisbane (including Chermside, Aspley, Nundah, Kedron, Stafford and Carindale), Moreton Bay (including Strathpine, North Lakes, Petrie, Kallangur, Mango Hill, Redcliffe, Deception Bay, Narangba, Burpengary, Morayfield and Caboolture) and Logan. We do not cover Springfield or Ipswich.

How to refer

1. Complete our online referral form, or call 0458 856 215, or email info@coreot.com.au.
2. Include the client’s details, funding type, reason for referral, discharge date (if known) and any risks we should know about.
3. We contact the client or family to book a home visit and keep you updated.

Common questions

How quickly can you see a client after discharge?

New clients are usually seen within about one week. Tell us the discharge date when you refer and we will do our best to fit in with it.

Do you see clients on Support at Home or Home Care Packages?

Yes. Support at Home replaced Home Care Packages, and we work with providers to see their clients for OT assessments, equipment and home modifications.

Can you see NDIS participants?

Yes, if they are self-managed or plan-managed. Core OT is not a registered NDIS provider, so we cannot see NDIA-managed participants.

Do I need a GP referral?

No. Hospital staff, aged care providers, families and clients can all refer directly.
