Memory changes can make familiar tasks and places harder to manage. Our occupational therapists visit people living with dementia or memory changes at home, and work with them and their families to keep daily life as safe, familiar and independent as possible.

Supporting carers

Caring for someone with memory changes is demanding. We include family and carers in planning so the strategies fit your household and are realistic to keep up.

Funding we work with

• NDIS participants who are self-managed or plan-managed (we cannot see NDIA-managed participants)
• Support at Home clients (formerly Home Care Packages), arranged with your provider
• DVA-funded clients
• Private clients

Where we visit

We are a mobile service. We come to your home, school, workplace or community setting across Brisbane North (including Chermside, Aspley, Nundah, Kedron and Stafford), Carindale, Moreton Bay (including Strathpine, North Lakes, Petrie, Redcliffe and Caboolture) and Logan. We do not cover Springfield or Ipswich.

Common questions

Can Support at Home funding be used?

Yes. Support at Home clients can access OT through their provider. We work with providers to arrange visits and reports.
